Below, you will find some important forms needed for medications, updates on health information, and school physicals. Feel free to download or print copies as needed.
Health Office Forms:
Dental Health Forms are requested (NOT required) when health appraisals are required (See Health Appraisal).
Health Appraisals are required in grades Pre-K, K, 2, 4, 7, 10, CSE re-evaluations, and new entrants.
Please be aware that a medication form must be signed by a doctor and a parent. This form must be kept in health office for all prescription and over the counter medications given at school.
