Individual Student Log

Use this log sheet for one student participating in one event of community service. 

Students should complete the form in their own words and their own handwriting; just like their homework. The supervisor should only be asked to sign it. 

Save the document to your device and then print it or email it to schase@pval.org. Paper forms must be returned to the Community Service Coordinator in the HS Counseling Office.

Use this sheet for multiple students participating in one event of community service.

Hourly Time Sheet

Use this time sheet for one student participating in an ongoing event of community service (for example: volunteering weekly at the library). Please turn in this form after all time slots are filled (even if the event is still ongoing) or after completion of the event.

It is required that this time sheet be attached to the Individual Student Log sheet. Hours will not be considered without the Individual Student Log.
